Set $wgShowExceptionDetails = true; at the bottom of LocalSettings.php to show detailed debugging information.
On a side note, can someone actually set $wgShowExceptionDetails to True so we can post more detailed error messages? And can someone add “Please open a bug report in the forums” to the error message?
By the way, I think I figured out the issue. The problem is that image uploads by new users keep on appearing as unpatrolled in Recent Changes. I think the way that WMF has made RC patrol is to patrol ALL recent changes in all namespaces, but they just have only new pages be patrollable. So maybe we can do the same - have a separate image patrol section of recent changes patrol so we would not have to file dozens of bug reports for the same error.

, we’ve been aware of this issue since it first started happening - that’s why the original reports are still open. I don’t think it’s quite that easy to fix, but the team are on it! I’ll pass on your feedback. Thanks :)
Is the issue fixed now?
Also, can you consider making that configuration change I requested where you set $wgShowExceptionDetails = true; so we can make inferences on what is wrong?
I made a post on this error also, although I’m unsure which one was posted first.
