Mr. Walker,
I am contacting you regarding the behavior of a fellow employee, John Emerson. Soon after John began working here this past July, he started to cause problems for me, other members of the team, and even some of our customers. While I initially thought these were isolated incidents, I have noticed a pattern in John’s behavior that is negatively affecting the entire store.
Attached to this letter is an incident log of five different instances over the past two months that demonstrate the problems John is causing. Any one of these things by themselves would not be a huge issue, but these five incidents are indicative of a much more widespread history of bad behavior. I am not alone in noticing these things. Several other members of the team have brought this up to me, but no one felt comfortable coming to you. Because I have been here longer than most of the other employees, I decided I should let you know.

I just felt the need to bring this to your attention, and I wanted to provide you with some specific examples to help you gauge the situation.
