So, you design your truss chords to meet over the wall, or what's called a cantilever. A cantilever extends the chords outside the but is braced to withstand load. The length depends on the total width of the house. Also, there are designs that are considered energy efficient by building in a small cantilever, and a vertical support over the wall, so at that point, you can have deep insulation. In a normal design, the insulation over the wall is only as high as the vertical thickness of the top chord.
