Keep the tongue relaxed down and forward. Practice singing vowels only, on a scale. Imagine the sound is in the middle of the mouth -- keep it out of the throat.

If it helps practice in private, it sometimes helps if no one is watching you. Also when you are approaching a high not take your forearms and put them in front of you then put one hand over the other in a circular motion go from slow and go faster the higher you get. Try leaning forward while doing this. You can also try whatever hand motions work best for you whether that be reaching or pushing or just whatever. The most important thing is don't let those notes intimidate you!
