If you mean default clothes, then you have to make sure the torso and legs are different colors. If you are on a phone or tablet, there may not be an "advanced" option in blue. This can be solved by going into a browser on your phone/tablet, logging in on roblox.com, and changing your avatar there. If your character doesn't update, there might be a "redraw" option to update your character.

There is no for sure answer since it is very rare for someone's account to get hacked. It might have changed back to default either because the person who got into your account changed it, or because Roblox reset your account to try and get the hacker out. My suggestion is to not give anyone your account password or e-mail, because with either of those things someone could get into your account. Even if something says they have free robux, don't give them anything because those are scams.
