Why do I keep dying?

Don't use the same tactic. If your opponents keep killing you, try to be less predictable. Here's an example: I'm playing nuketown on black ops 1. I keep going for the right side, but I keep dying. Then I go for the left side, and get a kill. Remember, use different tactics and in Call of Duty, timing is literally everything!

If you are peeking over an obstacle, or lying down, someone may be on the left/right or on top of something near your position. Someone also may have a thermal scope to spot you if you are in the bushes. But also, the spot you chose for your sniping may be in the enemy spawn!
