Is it worth unlocking it if you're not going for Real Platinum God?

The items are definitely worth the effort and pain of doing everything as the lost, as they are really strong, helpful, and might as well be OP. It is harder to unlock the lost in Rebirth but in Afterbirth you got 3 more challenges on the completion post-it note. It can be a pain but, like I said, worth it.

Do I actually need to beat the lamb?

No, but it is faster than the first option, taking about 4 different runs plus the retries to get the right items. Defeating the lamb is part of getting Real Platinum God, but if you're not going for Real Platinum God and you have time, then the first one is OK. None of the characters are unlocked by beating the lamb, so it doesn't really matter.
