After downloading the app, sign in with your existing Amazon account. After you successfully sign in, there will be two options: Cloud or Device. If you go to Cloud, you will see all of the books on your Amazon account. If you go to Device, you will see the books that are downloaded onto your iPad. To download books, go to Cloud and tap on the books you want to download. They will immediately start to download. They will then show up on Device, and you can read without a connection.

If your book was downloaded from the Kindle store, it probably wasn't downloaded to your iPad. Look for books not yet downloaded and you may be able to find it. Or you could go to the Kindle store and force download it from there. If you downloaded your book from an external source (say, as a .prc file from an open source library), then you'd have to download it again with your iPad from whatever open source you found it on.
