It would take a little longer, but if you play the games (mucking out the stall, cleaning the paddock, etc.), feed your horse, and have your horse go to work, you could earn XP slowly and steadily until you gain a new level.

Rarely. You do get some when you first join My Horse. They also may give gems away if you level up, but that requires you to level up, meaning you must care for your horse. So, in general, no, you cannot get gem without doing anything. In order to accumulate gems, you must spend real world money.
