The top 3 stings (E, B & G) keep flying out from the saddles when under tension - what am I doing wrong? The guitar has had fret-leveling and re-profiling carried out, and is in the final throes of being rebuilt.

When tightening the strings, hold the strings on the saddle until they are tight enough that they're not going to come off.

There is a wide variety of different floating bridges outside of the Floyd Rose, which means there is an assortment of different stringing methods. The easiest way to distinguish a Floyd Rose bridge is by looking under where the strings enter. Unless it’s a counterfeit a licensed Floyd Rose would always have a label indicating that it is a Floyd Rose. If not, there is a possibility that your guitar has an Edge Bridge, frequently found on Ibanez guitars. It’s the same method for restringing as with a Floyd Rose.
