Cool! I’d love to take it. For some reason, when I clicked on the link, it wanted me to sign into gmail. I think the survey must still be private. There’s probably a way to make it public so we all can take it.

One suggestion, though. I would make including email optional, or give the alternative option ‘I don’t want to give out my email, sorry’
