I apply serum, then moisturizer, then foundation. Why does everything turn to wet flaking?

Because the moisture you are putting into your face with the serum and moisturizer is good; however, the moisturizer dries to keep the moisture in the skin. Putting foundation on top of that makes it look cakey and dry, and it's probably not good for the skin. Instead, try just using concealer set with translucent powder.
