If you use it against Rayquaza, it won't work, because it is a flying type pokemon. Ground attacks won't work against flying types or pokemon with the ability to levitate.

Sure, it's just very hard. The chances increase if the enemy Pokemon is paralyzed or sleeping. And they also increase if you bring it to the lowest amount of HP possible (effectively with "False Swipe").
