    If you mean adding the name of the dog's registered kennel to its name, that does count as a name change. If your registration certificate does not include your dog's name, that most likely means that it has only been registered as part of a litter (probably by its breeder) rather than individually.     Then on file your dog just has your name. That is it, simply change the name on the file back to the dog's as you would change it normally.

You also need to mention your name, the dog's full name, gender of the dog, color of the dog, and the breed of the dog. You also need to give a $20 correction fee. They will send you another registration certificate with the corrected name.
