    You need to have a heart-to-heart with your ex. Tell her that you were really hurt when she left you, and that while you still have feelings for her, you're afraid of being hurt again. (This is completely valid.) Use "I" phrasing to focus on your feelings, not her actions. Then hear her out. If you're not sure yet, say "I hear you and I need time to think." It's okay to feel hurt, scared, sad, and whatever else you might feel. It sounds like it was a difficult breakup for you. Consider seeing a counselor to help you work out your feelings, especially since it was so devastating in the past. Suicidal thoughts are serious, and counseling is a preventative step to keep this from coming back again.
