If they say you do, you can submit an affidavit of nonliability for toll in the mail. Look the form up online; it has the address you need on it.

That's a great question. They snap photos of both front and back license plates. If you have a front plate, you're probably better off putting the truck's plate. If you drove from a state with no front plate, the horse trailer is probably the proper number. You may need to make two separate accounts just to be sure.
