    Check that the ground is leveled and pitched after you've excavated the area for your patio. Also, keep in mind what direction the water is going in relation to your patio, and think about which direction you'd like the water to go.

    Yes. Just use an extra 2 inches of concrete so you have a total thickness of at least 6 inches.

    The size of the gravel is not as important as packing it down. If the gravel is packed down, it will prevent the cement from cracking. If the gravel is not packed down, it will settle and the cement will crack where there is no support. Any gravel larger than 1/4 inch will pack down well.

    Yes, but the trick is to get the concrete to a consistency that it doesn't stick to the box and apply some kind of oil to the box beforehand to get clean edges.

    You can buy a groover, it's a special trowel. It will make its own cut for you without you having to worry about measurements.
