If you’ve kept a duvet cover on it and there's no obvious need for washing (spilled liquids for example), then you should not need to wash it more than once a year. Even then you may just spray it with vinegar and put it in the dryer to kill the dust mites and freshen it up.

No, you will need to remove your duvet from its duvet cover in order to wash it. Follow the steps listed here to wash your duvet. Your duvet cover should be much easier to wash because it is smaller and doesn't contain down or down alternative filling like the duvet itself. You can usually wash the duvet cover in your home washing machine on a regular cycle. (Be sure to check the label for washing advice for your specific cover.)
