Ask his teachers to keep a 'communication diary'. The teacher will write down how he behaves in school each day and send the diary home with him. Disicipline or rewards can be presented at home depending on how he's behaved. You can communicate back to the teachers what steps you're taking at home so that everyone is on the same page. You may also want to explore the cause(s) of this behavior a little: Is he lonely at school? Is he homesick or scared? Are children being mean to him?

Before entering the hospital, get down on his level and explain in words he can understand that he needs to use his "quiet voice" while inside. Have him practice with you what that would sound like until you're satisfied he gets it. Make sure that you have toys or a tablet to entertain him -- a bored child is more likely to be noisy. If he starts making too much noise inside, firmly pull him aside and reiterate that he needs to use his quiet voice.
