I was looking at Special:RecentChanges
, and I noticed this User rename log listing come up:

As you can see, the rename log reveals the IP Address of the user who created the account. Normally, only CheckUsers should be able to see this. I think that the IP Address should not be visible in the user creation log. Maybe the original username could be used in the log instead of the IP Address.

This has been updated so the IP address doesn’t show. I’ll keep an eye out over the next few days to make sure it’s working as expected. Thanks R2.
