It seems that a new update has made it so that whenever an article gets nominated for deletion, it automatically gets moved to quality review. This causes an issue with the New Article Boost Tool since once you nominate an article for deletion in it, it will automatically demote it, which causes the article to be demoted twice. Here is a screenshot of the Recent Changes page where this has happened today:
As you can see, articles that get nominated for deletion from the New Article Boost tool get demoted twice, which results in them getting the category tag twice.
The way around this bug would be to manually go to the article page and then adding the NFD tag through that whenever you want to nominate an article for deletion, and then exiting out of the article from New Article Boost without demoting it. This is annoying though.

Ooh. That cant be good, seeing that the edits you found were coming from my traditional actions in NAB.
