Sorry, I don’t know if this is actually a bug, but have you disabled Special:WhatLinksHere? I tried typing it in manually to find where an image had been used on User_talk pages (a user had put it in an unclosed div block), but got a 404 error.
To clarify: A user had *repeatedly* put it in obviously pre-formatted nested div blocks, but hadn’t closed them properly. I was trying to figure out where else he might have done the same.
Hmm… maybe I was using the wrong format when I was typing it in. And it works when I try a page in the article namespace. But it still won’t give me any results for the image page, although it otherwise seems to be processing it correctly. I also tried with another image, and got the same results. Maybe it’s disabled for images? I notice that the image pages lack the usual (for MediaWiki) list of pages they’re used on.

This doesn’t work anymore, but you should be able to see a list of the pages that use the image on the image page itself, with the heading “More images from…”
