Reshiram has a low catch rate. Keep trying. Even if you're out of Ultra Balls, an original Pokeball just might work.

Would I need both Reshiram and Zekrom to catch Kyurem? If so, do I go back to the same place I caught Reshiram/Zekrom to encounter it?

Yes, you need both Reshiram and Zekrom. If you have both these Pokèmon in your party, a new area (the Gnarled Den) will open up behind Mt. Chimney, and Kyurem can be found there.
