Took screw from cap, and cover will still not come off?

Try spraying a lubricant like WD-40 into the connection joints, then let it penetrate for 5-10 minutes before trying again. If it still won't budge, contact a plumber so you don't damage the faucet or sink.

Do I have the tap fully open or closed to change the tap valve?

I usually have the tap half open. If the tap is firmly off it applies pressure to the threads on the tap body making it hard to undo, and if you have the tap closed when you reassemble then you might not get the tap body fully screwed on leading to leaks.
