    That depends entirely on which part is broken - key rings and metal parts can always be replaced, but new hama beads are difficult to fuse to already ironed ones - you will likely just end up with a really weak and fragile bond. You can always try with various kids of glues, but its usually easiest to just re-make the piece.

    Yes. It's in fact highly recommended for strength and durability especially if you're making a key chain which will inevitably go through some wear and tear.

    I'm pretty sure on Perler.com is says that you are supposed to leave the holes when you iron them, otherwise you're melting them too much. The finished project looks different when melted without holes.

    It's all up to personal preference really, melting them more will give more strength up to a certain point, but yes as well designs usually look better with a uniform appearance of holes. Try a few different irons and see what works for you.
